I, the player, and the parent/guardian of the above-named individual, acknowledge that participation in athletic events involves the risk of physical injury and that protective equipment does not prevent all injuries. 

I do voluntarily assume all risks and hazards incidental to my child’s participation in the Windsor Stars Baseball Association, including any associated with the use of equipment used during training. I hereby waive, release, absolve, indemnify, and agree to hold harmless the Windsor Stars Baseball Association, coaches, assistants, and persons from any claims arising out of injury to my child.

I further grant Windsor Stars Baseball Association the right to name and photograph / videotape facilities, activities, and program participants for publicity, promotion purposes, or publication in local media services.

I agree to abide by the decisions and policies of the Windsor Stars Baseball Association
Board of Directors and authorize the release of information on this application to Baseball Ontario, or 3rd parties expressly for insurance purposes.
